We provide Patient Centered Medical Home model of care.The PCMH is a health care setting that facilitates partnerships between individual patients,and their personal physician,and when approprite,the patient's family.The medical home is responsible for all the patient's health care needs or taking responsibility for appropriately arranging care with other qualified professionals.The personal physician leads a team of individuals who collectively take responsibility for the ongoing care of patients.Evidence based medicine,health information,clinical decision support tools guide decision making to support pateint care,patient education.This improves the efficiency of healthcare system and ultimately improves health outcomes.
